Review Summary
This is a great effort, and the logic for creating labels and placeholders is spot on! However, changes are needed regarding the final placement of the labels.
Currently, you're using appendChild, which adds the new <label> as the last child of its container. This results in the label appearing after the input field, which doesn't match the visual layout in the task's example screenshot where the label is above the input.
To fix this, the label needs to be inserted before its corresponding input. You can achieve this by using the insertBefore method on the parent element. This small change will correctly position the labels as required.

While appendChild correctly adds the label to the parent container as instructed, it places it as the last child. In a standard document flow, this would render the label after the input field. To match the visual result in the screenshot where the label appears above the input, consider inserting the label before the input element. The insertBefore method on the parent element would be suitable for this.
