V0.0.2: add effective inserts and deletions

Delta-notifier counterpart of this pr.

Forward effective inserts and deletions to asking clients.

Adds following options:

• matchOnEffective: match on effective inserts and deletions
• requestPerMuCallIdTrail: deltas are folded with potentially multiple muCallIdTrails, send one request per muCallIdTrail

This also seems to include the "index" field, in which case #7 can probably be closed in favor of this?
