Summary
Wires classifier-error policy through backend dispatch, HITL review resolution, stats, notifications, SDK enforcement, dashboard controls, and operator docs. BLOCK mode can now fail closed on classifier errors, HITL can hold actionable classifier errors for review, and the dashboard exposes classifier errors separately from MAD findings.
