Doc and test follow-ups from the ethereum#3126 review: document all three ACCOUNT_WRITE exemptions and the pre-state creation-target check; pin the failed-create floor expectations to post-ethereum/execution-specs#3144 header semantics and add a floor-binding revert arm with regime asserts.
