- Adds a Codex-style default "codra" terminal entrypoint.
- Adds "codra init" for CODRA.md and .codra project setup.
- Adds "codra doctor" environment checks.
- Keeps existing JSONL task protocol working.
- Updates npm CLI quick-start docs for pnpm/npm global install.
- Does not publish to npm.
