Adds an optional field to the RegisterRequest to upload a CSR.

Questions/TODO:

  • are any other fields required for CSRs? (waiting for Sectigo to reply to support ticket)
  • is a pkcs12 password still required for CSRs?
  • is there anything else we should return in the RegisterReply?
