feat(eve)!: unify public session operation APIs

Summary

• combine the fixed-ID session handles and the breaking public migration into one implementation PR
• expose immutable-ID Session handles through attachSession(sessionId) and client.sessions.create/attach
• keep the singular HTTP surface: create with POST /eve/v1/session, follow up with POST /eve/v1/session/:sessionId, and address controls and streams by that same session ID
• expose custom-channel from(address) sources and top-level resolveSession(address); keep ChannelAddress internal
• use positional send(message, options) and separate respond(inputResponses, options) on channel sources, fixed sessions, TypeScript client sessions, and eval sessions
• expose cross-channel and schedule to(channel, target).send(message, options), with channel references enforced at compile time
• expose flat Slack ctx.send(message, options) and ctx.respond(inputResponses, options) while preserving the existing Chat SDK bridge API
• keep session identity off channel event context: normal handlers use ctx.session.id, while session.failed exposes data.sessionId
• make canonical eve onMessage always dispatch or fail, removing the unparseable HTTP 204 drop outcome
• expose channel-local continuation tokens that round-trip through from(), resolveSession(), and rekey() without framework namespace prefixes
• make message and inputResponses mutually exclusive across protocol, client store, and HTTP validation
• expose public session callbacks without leaking internal TurnCaller routing and remove the obsolete remote-deployment compatibility shim
• align cancellation across direct, channel, HTTP, client, ACP, TUI, and remote-agent surfaces as { status: "accepted", sessionId } or { status: "no_active_turn" }; accepted HTTP cancellation returns 202
• expose stable server error codes through ClientError.code, including session_not_active
• migrate built-in channels, clients, framework adapters, fixture evals, tests, docs, and the changeset without a compatibility layer

Dependency

• refactor(eve): unify durable session command routing #1586 — durable command runtime, now merged into main
• this PR — session handles plus the sole breaking public migration
