B2: remove adaptive no-op control plane and split residue

Follow-up subtraction batch to #26, sourced from the post-campaign blind rescore (D5 lead: adaptive no-op control plane).

• B2-1 remove adaptive no-op hooks (selection/final-projection/target-tracking) and their loop call sites
• B2-2 delete isolated helpers (_merge_pressures, _profile_default)
• B2-3 remove loop/ split leftovers (unused imports, stale skeleton docstrings)
• B2-4 remove duplicate _restore_paths call in prompt_view
• B2-5 align gate docs/comments to advisory (answer-first) reality
• B2-6 prune dead adaptive config (mdl / elite_gate / non-observe spatial modes)

Net −115 lines. No gate behavior change: hard-reject surface stays exactly structurally_blocked + failed-patch; sanctioned advisory surfaces (evaluator summary, challenge memory, verification cache, spatial observe, final certificate) untouched. Full suite 700 passed / 1 skipped (independently rerun).
